Progress Reports must be submitted within 30 days following the June 30th and December 31st Progress Report schedule.

Please download the following file and use for reporting the use of Core Agency Support.
Core Agency Support grant recipients will be required to submit one final report following expenditure of all JHF funds. Typically, core agency support funds are expended within a 12-month period after receipt of the grant award. Requests for grant expenditure extensions must be approved by Foundation staff.
If you have any problems or questions about these forms, please call Rachael Gordon at (913) 327-4623 or email firstname.lastname@example.org
IMPORTANT DETAILS RELATED TO PROGRESS REPORTS:
All unexpended grant funds must be returned to the Foundation with a reference to the Grant Number and the date of your final report.
Requests for extensions to complete Program grant activities using unexpended grant funds, or requests to use line item funds for grant related purposes other than outlined in the grant application, must be submitted in writing and discussed with Foundation staff before the end of the grant period.
Agencies with late reports will not be considered for future funding until all overdue reports are received.
